General-purpose filter design for neural prosthetic devices
Brain-driven interfaces depend on estimation procedures to convert neural signals to inputs for prosthetic devices that can assist individuals with severe http://www.easymedonline.com/Mobility-Scooters-s/20.htm deficits. Here we report a coherent estimation framework that unifies these procedures and motivates new applications of prosthetic devices driven by action potentials, local field potentials (LFPs), electrocorticography (ECoG), electroencephalography (EEG), electromyography (EMG), or optical methods. The brain-driven interface is described as a probabilistic relationship between neural activity and components of a prosthetic device that may take on discrete or continuous values. A new estimation procedure is developed for action potentials, and a corresponding procedure is described for field potentials and optical measurements. We test our framework against dominant approaches in an arm reaching task using simulated traces of ensemble spiking activity from primary motor cortex (MI) and a power wheelchairs navigation task using simulated traces of EEG-band power. Adaptive filtering is incorporated to demonstrate performance under neuron death and discovery. Finally, we characterize performance under model misspecification using physiologically realistic history dependence in MI spiking. These simulated results predict that the unified framework outperforms previous approaches under various conditions, in the control of position and velocity, based on trajectory and endpoint mean squared errors.

Effects of respiratory resistance training with a concurrent flow device on wheelchair athletes.
BACKGROUND/OBJECTIVE: To determine the effect of respiratory resistance training (RRT) with a concurrent flow respiratory (CFR) device on respiratory function and aerobic power wheelchairs, http://www.easymedonline.com/Mobility-Scooters-s/20.htm athletes. The RRT group performed 1 set of breathing exercises using Expand-a-Lung, a CFR device, 2 to 3 times daily for 10 weeks. Pre/posttesting included measurement of maximum voluntary ventilation (MVV), maximum inspiratory pressure (MIP), and peak oxygen consumption (V(O2peak)). RESULTS: Repeated measures ANOVA revealed a significant group difference in change for MIP from pre- to posttest (P < 0.05). The RRT group improved by 33.0 cm H2O, while the CON group improved by 0.6 cm H2O. Although not significant, the MW increased for the RRT group and decreased for the CON group. There was no significant group difference between V(O2peak) for pre/posttesting. Due to small sample sizes in both groups and violations of some parametric statistical assumptions, nonparametric tests were also conducted as a crosscheck of the findings. The results of the nonparametric tests concurred with the parametric results. CONCLUSIONS: These data demonstrate that 10 weeks of RRT training with a CFR device can effectively improve MIP in wheelchair athletes. Further research and a larger sample size are warranted to further characterize the impact of Expand-a-Lung on performance and other cardiorespiratory variables in wheelchair athletes.

Reliability of upper extremity anaerobic power assessment in persons with tetraplegia.
BACKGROUND/OBJECTIVE: Reliable assessment of upper extremity anaerobic power in persons with cervical spinal cord injury (SCI) may indicate the ability to successfully and safely perform many daily activities. PURPOSE: To examine test-retest reliability of upper extremity Wingate anaerobic testing (WAnT) in persons with motor/sensory complete tetraplegia. METHODS: Forty-five persons with cervical-level SCI (15 individuals each at C5, C6, and C7 levels of injury) performed 2-arm WAnT bouts, with 2 to 4 days between bouts. Subjects performed the WAnT seated in their medicare power wheelchairs and medical scooters using a tabletop-mounted Monarch 834E ergometer. Resistance loads were applied relative to injury level, with 1%, 2%, and 3% of body mass applied to subjects with C5, C6, and C7 level injuries, respectively. All subjects were directed to crank the ergometer at maximal velocity for a 30-second period. Values of peak power (Ppeak) and mean power (Pmean) were determined using an SMI OptoSensor 2000 system. Ppeak and Pmean were compared between trials and between groups using 2-way analyses of variance for repeated measures. Coefficients of determination (r2) were calculated between trials. RESULTS: There were no significant differences in Ppeak or Pmean detected between the 2 trials of WAnT in the C5, C6, or C7 groups. Regression analyses revealed statistically significant associations between bouts for Pmean and Ppeak in each of the 3 groups (P < 0.05). CONCLUSIONS: Upper extremity WAnT is reliable for upper extremity anaerobic power assessment in persons with cervical SCI at or below the C5 level.

Unilateral neglect is a challenging disorder that pervades a range of behaviours following stroke and hampers recovery. Results from the obstacle course confirmed the clinical observation and predicted bias of contralesional errors. However, the second task revealed a robust “crossover” effect. Patients deviated to the ipsilesional side for large gaps but deviated increasingly contralesionally when steering through small gaps in behaviour that was analogous to that previously shown on line bisection tasks. Contrary to being seen as an unintuitive finding, further analysis of these errors suggests that patients are giving disproportionate weight to the location of the ipsilesional object when plotting a midline course between two objects. Our results provide a platform for further studies to investigate the modulation and rehabilitation of this important skill.

Monitoring seat feature usage among wheelchair users.
To help manage pressure ulcers and adjust posture, power wheelchairs can be equipped with seat tilt and backrest recline features. Clinicians usually prescribe these features for wheelchair users who are vulnerable to pressure sores or have difficulty changing postures, but little data has been collected to determine whether or not, and what extent, these features are used. Satisfaction with technological equipment in individuals with tetraplegia following spinal cord injury
OBJECTIVE: To measure the acquisition, use and satisfaction with high technology equipment by spinal cord injured tetraplegic subjects. DESIGN AND SETTING: A questionnaire was mailed to 102 tetraplegic subjects who were hospitalised in the rehabilitation center of Kerpape (Ploemeur, France) between 1998 and 2004, and 59 subjects responded. The questionnaire asked about the use of telephones, computers, power wheelchairs, medical scooters and environmental controls at home. RESULTS: When a piece of equipment was acquired, it was very often used. Patient satisfaction with equipment was 79.3%. Home phones and mobile phones were often used with options such as hands-free devices (78 and 59% respectively). A total of 64.4% of subjects acquired a manual wheelchair and 61% a power wheelchair. The most commonly acquired options on the power wheelchairs were the powered recline (73,7%) and tilt (71,1%) systems. All options were used but all were more desired than acquired. A total of 27.1% of subjects desired a pushrim-activated power-assist wheelchair, but only 15.3% had acquired one; 695% of subjects had a computer. Communication was the first use for the computer (82.5%); 49.2% of subjects had acquired an environmental control system, but 20% desired one. The first reason for lack of acquisition was financial difficulties but also accessibility and information problems. The factor that influenced the acquisition and need for equipment was the degree of spinal cord injury. No other factor reduced patient satisfaction with equipment. CONCLUSION: Patients were satisfied with the equipment they acquired. But their needs, especially wheelchair options and environmental control systems, were not satisfied.

A preliminary study on the impact of pushrim-activated power-assist wheelchairs among individuals with tetraplegia
OBJECTIVES: This preliminary study was to evaluate the impact of pushrim- activated power wheelchairs (PAPAWs) on mobility, community participation, satisfaction, and psychosocial impact among individuals with tetraplegia. DESIGN: Fifteen manual wheelchair users with tetraplegia completed a 4-wk protocol including a 2-wk trial where they used their personal wheelchairs for mobility and a 2-wk trial where they were provided with a PAPAW and could choose either wheelchair for mobility. The mobility levels with both wheelchairs were recorded by a datalogger. Participants completed daily questionnaires regarding community participation and medical scooters their satisfaction with the wheelchair used. The Psychosocial Impact of Assistive Devices Scale was used to compare the psychosocial impact of PAPAWs with their personal wheelchairs. RESULTS: Participants chose to use PAPAWs and their personal wheelchairs at a similar frequency in the PAPAW trial. The distances traveled with both wheelchairs were not statistically different (P = 0.09). Participants traveled significantly faster with PAPAWs (P = 0.03). PAPAWs did not result in significantly greater community participation, satisfaction, and psychosocial impacts than the personal wheelchairs. CONCLUSIONS: PAPAWs could be a viable mobility option for individuals with tetraplegia, which can provide independent mobility especially for outdoor activities. A user’s preference, life style, physical conditions, transportation issues, and environmental factors should be considered in prescribing such a device.

Handicap needs awareness
I would like to raise people’s awareness of www.easymedonline.com needs around the Laboratory workplace. People on crutches, scooters and wheelchairs have some different needs than the ambulatory. I am the first to admit that when I was ambulatory I was unaware of the environment around me in these terms. However, now that I am in a wheelchair, I would like to share with my co-workers some needs and easy-to-do things for people around the Lab.
Doors are a big issue. People in power wheelchairs, scooters etc., need to place themselves to the side of the door to open it. Objects such as trash cans, tables and chairs placed next to the door result in a situation where a disabled person cannot open the door. Please make sure that a 3-foot-wide space is available between the side of the door that opens and any object placed along the wall.
Another issue is bathrooms. If there are multiple stalls available please do not choose the wheelchair accessible stall if you do not need it. It is frustrating and potentially unpleasant to have to wait for the bathroom stall because it is occupied when three other stalls are available.
Wheelchairs, scooters and crutches require more maneuvering room than just the typical human body. Objects piled in hallways can create impassible situations. Imagine not being able to go to a meeting or your office because the hallway was blocked and there was no way around.
Even though it seems like sitting in a wheelchair or scooter requires no effort, disabled people are often plagued by fatigue. And, little tasks such as opening doors can require a great deal of effort. Think about how we would feel to go about the day with a bungee cord around our legs and arms.
I want to thank people for spending an extra second to make life more normal for people who are disabled.
